About the Project

Rheumatic heart disease (RHD) affects more than 33 million people globally and leads to >300,000 deaths per year, almost all at a prematurely young age, and in settings of poverty. There is renewed focus on RHD, highlighted by adoption of the Global Resolution on Acute Rheumatic Fever (ARF) and RHD by the World Health Assembly in 2018. At present, secondary prophylaxis in the form of 3- to 4-weekly intramuscular (IM) benzathine penicillin injections are the cornerstone of RHD control, and the only proven approach that is both therapeutically and cost-effective. The overall management is limited by poor adherence to painful IM injections given on a monthly basis. Consequently, the majority of people with ARF or RHD do not receive adequate secondary prophylaxis.

Against this background, the proposed research program will comprise part of a suite of clinical studies, including an NHMRC-funded project [#1187743] investigating the effect of penicillin in a controlled human infection for Streptococcus pyogenes, and a study of subcutaneous infusions of penicillin in RHD. Using a combination of analytical and pharmacokinetic modelling methods, this research will evaluate penicillin for novel clinical and research applications, to inform further clinical studies in Australian settings and international collaborators’ institutions
