About the Project
Intermediate care (IC) is a relatively new patient care setting within the UK. Essentially, patients are transferred to IC from an acute hospital, prior to either discharge home or to long-term residential care. All patients over 18 years of age are eligible for transfer to IC, however, the majority of patients in IC are older patients, i.e. 65 years or older.
The primary functions of IC are to facilitate recovery from acute illness, prevent acute hospital admission and maximise independent living. As IC patients are generally older patients, and are therefore likely to be prescribed multiple medicines, medicines management within these settings is important.
Whilst IC is multi-disciplinary, there is anecdotal evidence suggesting that pharmacists’ involvement in these facilities is minimal. The purpose of this project is to explore pharmacists’ roles in medicines management within these facilities, with particular reference to the supply and storage of medicines, both the admissions and discharge processes and the appropriateness of medicines prescribed to these patients.
The project will involve a mixture of qualitative and quantitative research methods, with subsequent triangulation. In addition to developing research skills, the researcher will also build on and develop clinical pharmacy skills in relation to pharmacotherapy in older patients.
A successful outcome to this work will be to develop a medicines management pathway for older people throughout their stay in IC.
